내달 1일부터 연 365회를 초과하는 외래진료를 받으면 당초 20% 수준이었던 건강보험 본인부담률이 90%로 올라간다.

보건복지부는 30일 이 같은 내용이 담긴 '본인부담차등화'를 시행한다고 밝혔다.

앞서 정부는 지난 2월 '제2차 국민건강보험 종합계획'을 통해 필수 의료 분야 보상을 강화하고,로또 총합불필요한 의료 남용 관리를 강화한다는 내용을 골자로 한 건강보험 정책 방향을 제시한 바 있다.

이번 본인부담차등화는 해당 방향의 후속 조치다.국내 연간 외래이용 횟수가 높은 점 등을 감안해 합리적 의료 이용을 유도하고,로또 총합의료 과다 이용 시 본인 부담을 높이겠단 의도로 기획됐다.

외래진료 횟수는 매해 1월 1일-12월 31일을 기준으로 산정하되,로또 총합올해만 제도 시행일인 7월 1일부터 산정한다.

본인부담차등화가 시행되면 약 처방일수 및 입원 일수 등을 제외하고 연 365회를 초과(366회부터)한 외래진료에 대한 본인부담률은 현행 평균 20%에서 90%로 상향된다.

아동,로또 총합임산부,로또 총합산정특례자(중증질환자 및 희귀·중증난치질환자)로서 해당 질환으로 인해 외래진료를 받은 사람 등은 예외로 인정된다.다만 이 경우에도 경증질환으로 외래진료를 받은 경우는 365회에 적용된다.

이에 해당하지 않는 산정특례자 또는 중증장애인은 건강보험공단(이하 공단) 내 '과다의료이용심의위원회'를 통해 의학적 필요성 등을 심의한 뒤 적용 제외할 예정이다.

본인부담금은 외래진료 시 의료기관을 통해 납부하거나 공단에서 미납부한 본인부담금을 사후적으로 징수할 수 있다.

복지부는 "합리적인 의료 이용을 유도하기 위해 의료 과다 이용 시 본인 부담을 높이는 것"이라며 "한해 수백 번 외래진료를 받는 등의 불필요한 의료 남용을 방지하기 위한 최소한의 조치"라고 밝혔다.
